Wellness Coach (Case Manager)
Services for the UnderServed (S:US) is seeking dedicated Wellness Coaches to provide case management services within the ATP Program in Manhattan. In this role, you will engage individuals with mental illness, assist them in community living, and support their recovery and independence.
Key Responsibilities
- Engage clients to support community living and housing stability.
- Teach necessary skills and link clients to medical, psychological, substance abuse, vocational, and other community services.
- Conduct group meetings on wellness management.
- Develop and review individualized service plans.
- Perform regular assessments and inspections of living spaces.
Qualifications
- Bachelor’s degree in human services or related field required.
- Minimum of two years’ experience working with individuals with mental illness.
- Proficiency in word processing and strong writing skills for developing treatment plans.
